Incident Record (Form IR1) - Portal
Guidance on the completion of the electronic version of IR1
In the event of an incident, that has the potential to cause or has caused harm, it is essential that an IR1 incident report is completed. This action will assist Ceredigion County Council to reduce accidents/incidents and manage the risks associated with our work activities.
Initiating the Incident
To initiate and incident report electronically enter the health and safety cardinet site and then click onto the Incident Record (Form IR1).
You will then be required to complete the 6 tabs:
- Where & When;
- Individual Involved;
- Circumstances;
- Equipment Involved;
- Action taken;
- Witnesses.
If an individual is involved two additional tabs will need to be completed:
- Physical Injury;
- Attention received.
You can save the information you enter at any time and leave the system if necessary.
When you have completed all the tabs press the submit button and the details of the incident will be on the screen. You will now be asked to press the Confirm button.
Now complete Person Reported to. You can enter the name of the person and press the search button. That person will be identified in the Search Box. Click on the name and you will then have a choice of Change or Submit. If you are satisfied with the form press Submit button.
The next screen will present two choices Change or Investigation. This allows a person who is entering the incident details and with the responsibility for investigating the incident to click on investigation and complete. If you are not the investigating officer just leave the system. The system will generate an email with the IR1 details to the investigating officer.
Investigating the incident
On receipt of the email containing the IR1 details click on the details to get into the system. Now click onto the IR1 number and you will be presented with 10 tabs for completion:
- Person Involved;
- Client fall;
- Facts;
- Response;
- Cause;
- Action;
- Comments;
- Investigation;
- Attachments;
- Notified.
In the Person Involved tab you will be required to click on, in the drop down list next to Period of absence, the duration of absence and it is essential that this is completed. The rest of the tabs completion is self-explanatory.
The Actions tab will require you to enter a priority for an action (High, Medium or Low) and the name of the person responsible for implementing that action.
The Attachment tab allows a file to be attached to the IR1 form. To do this click on Attachments tab and then click on Upload File. You can now Brows for the file. Once you have identified the file click on the Upload button and this will return you to the Attachment tab. You can then click onto Attach File, or, if you are not satisfied Cancel.
Once all tabs have been completed press Submit. The screen will then provide all the details of the investigation if you are satisfied press the Submit button.
A PDF file will now be sent to the person who entered the incident report and the Corporate Health and Safety Unit who will complete the form.
